Job DescriptionLOCATION: Kings Lynn – With multi site working around West Norfolk
JOB TITLE: Nurse
SALARY & BENEFITS: £14.24 - £15.76 dependent on experience
WORKING HOURS: 20 hours per week (flexible)
THE COMPANY: We are currently working with an NHS partnership which provides primary care services for over 37,000 patients at six Norfolk health centres. They are now seeking a Nurse to join the friendly team who is a Diabetes Specialist or someone who wants to train in this area.
THE ROLE: You will be responsible for providing a high quality nursing service to patients across all 6 practices. Your focus will be to support patients to be healthy and to monitor long-term conditions. You must be able to work collaboratively with the general practice team to meet the needs of patients and provide nurse leadership as required.
Key Responsibilities for the Nurse role to include:-
Provide specialist knowledge and care to patients in the Practice
To act as first point of contact to patients who have need to access the service
Plan care for patients, organising appointments and treatments
Will undertake a range of duties to ensure the effective delivery of care including undertaking specialist procedure, providing specialist advice, structured education and ongoing support to the patient group and their families on an individual need basis
THE CANDIDATE: The successful candidate will need to be a caring individual with excellent communication skills. Ideally you will be CHD/Diabetes trained, however full training will be given to the right individual. You will also need to be a reliable team player with excellent attention to detail.
Key experience essential for the role of the Nurse:-
Must be a registered Nurse with PIN
Live within a commutable distance to Kings Lynn and be able to travel to and from the 6 x practices
